Items Needed at Registration

• Legal Documentation (Original Birth Certificate and Social Security Card)

• Completed TUITION AGREEMENT (including PIP clause)

• Completed FACTS form (Automatic Tuition Payment Agreement)

• A voided check (savings deposit slip with account number or credit card number)

• Registration $210 fee (After March 7th $260)

• Medical forms *It is mandatory that all students have on file in the school office a current FORM DH 3040 / FLORIDA DEPARTMENT OF HEALTH SCHOOL ENTRY EXAM and FORM DH 680 / FLORIDA DEPARTMENT OF HEALTH CERTIFICATION OF IMMUNIZATION.  These forms are obtained from students' physicians and must be received prior to the student attending classes.
